Teddy Bear Birth Certificate

Officially name a new teddy bear with a printable teddy bear birth certificate. You can print out your teddy bears certificate of birth in pink, blue, yellow or grey. 

Each teddy bear birth certificate has space to add your teddy bears name and their date of birth as well as some boxes to add features such as fur color, eye color, length, weight, species and gender. 

Each teddy bear birth certificate requires a signature to show that you promise to give your new friend lots of love and a happy home. 

Included in this printable pack are also teddy bear adoption certificates. These are the same as the teddy bear birth certificates except they say adoption certificate across the top instead of birth certificate. They also have a space to say who adopted the teddy bear in place of the date of birth.
